BEFORE SURGERY
After a suitable disease history and physical examination, a road map for surgery is drawn, such as the shape of the nose, skin structure, relationship with the surrounding tissues, disorders that prevent breathing, and additional procedures that can be performed. The shape of the nose can be planned together with the patient in the computer environment by taking photographs of the patient. However, it should be noted that the accuracy rate of this image is not high. Conditions that may pose an obstacle to the operation are inspected by the anesthesiologist, and necessary blood tests are performed. In case of chronic diseases that may affect the surgery, additional examinations or consultations may be requested. If any blood thinners are used, they should be discontinued at least 1 week in advance. Smoking can often impair the wound healing process and comfortable breathing. There is no need to discontinue drugs used for other chronic diseases.
SURGERY PROCESS
Rhinoplasty operation is performed in a hospital environment and under general anesthesia. Although it varies according to the difficulty level of the nose and the application of additional procedures, it takes an average of 1.5 – 2 hours. There are two main techniques, open and closed. The main difference between these two techniques is that there is a 0.5cm long incision under the nose. No procedure is applied to the skin structure on the nose during rhinoplasty surgery. Problems such as skin structure, thickness or staining on it are not affected by this surgery. The procedure is about shaping the cartilage and bone structure, correcting the curvatures that affect breathing, and removing the excess areas. The nasal skin, which is removed after all these procedures, is covered again on the structures that have been shaped in the same way. At the end of the surgery, a plaster is placed on the nose. Except in rare cases, no tampons are placed in the nose.
POSTOPERATIVE
After the operation, food is eaten at the 4th hour and the patient gets up from the bed and starts walking. After the operation, ice is applied around the nose to reduce swelling and edema. Although swelling and bruises around the eyes are the most disturbing problem, they may not be in every patient. Past nose surgeries, large deformations, a hard bone structure increase the frequency of bruising and swelling. However, edema and bruises that occur are largely resolved within 48-72 hours. The patient is discharged on the same day. The patient returns to his daily life within the same week, and comes for the control after 1 week to remove the plaster and bandages. Despite the absence of a tampon, patients may not be able to breathe easily in the first days. At the end of the 3rd month, the swelling goes away by 80%, but the nose takes its real appearance exactly in the 6th month. For this reason, it is necessary to protect from impacts and avoid the use of glasses for the first 6 months.
PROBLEMS THAT MAY DEVELOP
Of course, every surgical procedure has its own complications. Although these are rare, they should be explained to the patient by the physician for the purpose of informing the patient. There are alternative solution options for all of the early and late complications that may develop. Temporary problems; loss of sensation, tenderness, pain, early problems; bleeding, infection, delayed wound healing, long-term problems; difficulty in breathing and aesthetic problems (asymmetries in the nose, collapse, etc.).
SECONDARY NOSE SURGERY
Patients who have undergone nose surgery for any reason before and are not satisfied may want to have the operation again. It is necessary to wait at least 6 months for a second operation in order for the nose to be in its final state and for all edema to pass. However, it should be known that secondary aesthetic rhinoplasty is a technically more difficult and troublesome process. In the first operation, the anatomical structure of your nose may be impaired, its flexibility may be lost, and the needed cartilage and bone tissue may be removed. In secondary nose surgeries, cartilage may be removed from the ear or ribs if needed.
